Hospital Registration

RegistrationInpatient and Outpatient Registration Procedures
Your registration process may include a contact with the Hospital’s pre-registration office. If you have any scheduling conflicts, please call our pre-registration and scheduling office, (573) 331-6351 or toll-free at 1-877-399-7123, between 7:30 am and 6:30 pm Monday-Friday.

When you register at the Hospital, a plastic identification bracelet will be placed on your arm. Please do not remove it. This bracelet serves as your identification throughout your stay.

During your hospitalization, you will be asked to provide information and sign forms which authorize Southeast to provide the treatment your physician has requested and to assign insurance benefits to the Hospital.

Registration forms should be signed by the person responsible for your bill. (For more information, please refer to the section about billing elsewhere in this booklet.)

If you are a beneficiary of the Medicare, Champus or Champ VA programs, please read the “important message from Medicare/Champus” provided to you upon registration.

Explanation of Hospital Costs and Insurance Coverage
Southeast Hospital will make every effort to help you understand the cost of your Hospital services and the coverage of those services by your insurance to the extent possible.

Before or during your registration, you may discuss these matters with a representative of our Patient Accounts Department who will provide you with the approximate cost of the services you are going to receive. This process can sometimes take a few minutes, so it is advisable to do this in advance of your scheduled service.

If you have questions concerning your insurance coverage, we suggest you contact your employer or insurance agent, depending on how your insurance is provided to you. These are the best sources of information about covered services, deductibles, and any co-payment requirements.

Patient Valuables
We strongly encourage you to leave anything which you consider valuable at home or send your personal valuables home with a family member as soon as possible after admission. Examples of personal valuables include (but are not limited too): jewelry (watches, rings, bracelet, necklace, etc.), cash, ATM/debit/credit cards, checkbook, clothing, etc. The Hospital will not be responsible for the loss or damage to any personal valuables while you are a patient.

If you are not able to send your personal valuables home with a family member, those items can be secured in the Hospital’s safe in the Patient Registration Department.

Lost and found items are kept in the Cashier’s Office on the first floor near the Lobby. If you lose something, check there to see if has been found.

Pre-Certification
Most medical insurance companies require that scheduled inpatient procedures, and some outpatient services, be authorized in advance. Many also require timely notification of urgent and emergency admissions as well. This pre-certification process is the responsibility of the patient or the patient’s family.

Failure to pre-certify may result in reduction of benefits or non-payment by the insurance carrier.

Information about your individual insurance pre-certification requirements may be found on your insurance card, or by calling your insurance carrier, agent, or your employer.

If pre-certification is required by your policy, you should make this contact as soon as an elective procedure is scheduled and within 24 hours, including evening and weekend hours, of an emergency or urgent admission.

Pre-Surgery Testing
If your next hospital surgical visit is planned in advance, take advantage of our convenient pre-surgery testing program through which your physician can schedule necessary tests prior to the date of your surgery.

On the scheduled pre-surgery testing day, you will report to Pre-Admission Testing & Surgery Registration located near the Emergency Department. During the pre-surgery testing visit, you will complete admission forms, receive special instructions about your surgery and a nurse will tell you the time that you should arrive at the Hospital on your surgical date.
