Diabetes Education Services at SoutheastHEALTH
It's easier to stay in control of diabetes when you have the right tools to handle it. That's where we can help.
Our center is staffed with certified diabetes educators, registered nurses and dietitians, and other diabetes experts who can teach you the latest information about the disease.
Here are some of the services we offer:
Individual diabetes counseling. You will work with a diabetes expert. After reviewing your health history and lifestyle habits, we will provide a personalized diabetes care plan.
Diabetes classes. Our three-month diabetes self-management program provides education in healthy eating, being active, monitoring glucose, taking medications, problem solving, healthy coping and reducing risk.
Medical nutrition therapy. Individualized nutrition education on diabetes, pre-diabetes, heart disease, weight management, kidney disease, metabolic syndrome, gastrointestinal disorders and other nutrition-related conditions.
Diabetes annual assessments. A yearly wellness visit is a great way to receive support and reinforcement of diabetes self-management goals.
Diabetes education at work and school. In addition to individuals and families, we help educate businesses and schools about diabetes.
Diabetes support group. Support group meetings are held the second Tuesday of each month, except for January due to possible inclement weather.
