Southeast HealthPoint Rehab provides a broad scope of rehabilitation services for adults and children. If you are facing medical issues, injuries or surgical recovery, our staff personalizes your treatment to obtain the greatest level of function and independence possible.
To assure quality care, therapists personally tailor and administer each phase of care as well as provide information and education.
Our state of the art facilities in Cape and Jackson are located adjacent to HealthPoint Fitness Centers, allowing to wellness services when you are discharged from therapy.
HealthPoint Rehab accepts referrals from physicians, physician assistants, nurse practitioners and dentists for physical, occupational and speech therapy services. As a patient, you have the right to choose your therapy provider. Our experienced staff and wonderful facility make HealthPoint Rehab a natural choice for your services.
Billing & Insurance
For your added convenience, billing and insurance services are handled onsite. We call to verify coverage of therapy services and file your insurance claims. If you have financial concerns, please speak to a member of our billing department. There may be options available to assist you.
What to Expect
At the time of your first appointment, be sure to bring the following:
- A driver’s license or picture ID.
- Your insurance cards. If filing with Workmen’s Comp you should also call prior to your appointment so we can pre-certify your visit with your case worker. If you have been in an auto accident or another liability situation, please bring information about who we should contact for payment.
- The prescription for therapy. Without this, we will not be able to do your evaluation.
Your first appointment will last approximately 45-90 minutes. Subsequent appointments will be made based on what the physician has prescribed and what you and therapist decide is most effective in meeting your goals after your evaluation.
Wear comfortable clothing. If you have a shoulder problem, you may want to layer with a sleeveless shirt. If you’re being seen for a knee or leg problem, wear shorts. We also have patient gowns and shorts available for use.
